DDP Hosts Transition Day for Families

Above, DDP case manager Natalie was ready to answer questions and help clients at Transition Day.

On Saturday, May 4th, in partnership with the Department of Developmental Services (DDS), the Walnut Street Center and other providers, our Developmental Disabilities Program hosted a Transition Day for families of children between the ages of 18 and 22. The event was held to support families and answer their questions about the inevitable period of transition when their children will move from receiving DDS children’s services to receiving DDS adult services. Families were provided with the information and resources necessary to support their child in the final years of school and to begin planning for their child’s future as an adult.

We were proud to partner with DDS and the Walnut Street Center for this event and look forward to holding more in the future! Informational events like these are necessary, as they are part of the comprehensive support we provide to the individuals and families we serve. Events like Transition Day allow us to help those we serve take full advantage of the resources available to them so that big moments of transition and change are easier to navigate.
